At Person Centered Care Services, our programs and services put people first.

We are dedicated to providing supports that not only enhance the lives of people with disabilities but also impact the communities they live in. 

Everyday, we support families of people with disabilities, we include those we serve in community activities to expand their skills and experiences, we engage members of the communities we work in through diversity and allyship trainings, and in all that we do, we are always advocating for equal access and opportunity for those who need it. 

Community Supports

We work closely with local businesses and community organizations to find volunteer opportunities that match the interests of the people we support.

From delivering meals to the elderly, for those who want to give their service, to learning firsthand from professionals about all that is required to do a specific type of job, the people we support discover a little about themselves and their passion(s) everyday.

Residential Supports

Having a place to call home is a basic right that all people deserve. Yet people with disabilities often go a long time without finding a place to live. Discrimination, among other things, often prevents or delays the process of setting up living arrangements.

With our Residential Supports, the people supported are able to live in a place they can call home, where they feel safe, independent, and supported by staff, family, and other loved ones.

Self-Directed Supports

Through Self-Direction, a person and his or her family can take control of the many components required to fund and maintain the services that are most valuable to them. Our Fiscal Intermediary and Agency Brokerage services make this possible by offering a wide range of supports to people and families who choose to self-direct their services.

Self-Direction gives the people and families we support the flexibility to choose the mixture of supports and services that are right for them, the staff and organizations that provide the services, and the schedule that works best for them!

Day Hab & Employment

Some people may find they need extra guidance and exposure to different environments before they feel prepared to transition to the next phase of their life. That's where Day Supports steps in, offering an experience tailored to personal interests!

Here, adults can forge meaningful connections, immerse themselves in their community, hone literacy skills, and acquire hands-on experience or a strong work ethic in areas that resonate with their interests or desire to make a difference.

Education & Training

A well-informed family makes all the difference in the quality of life that both children with disabilities and their family experience. Providing support for families ensures that they are not only able to understand how to give care to their loved ones but have all the necessary resources to do so.

Family Education & Training sessions provide families with valuable resources and information about service alternatives available to them and how to gain access.
