Intake & Eligibility
As many as 40% of New York City schoolchildren with disabilities do not receive the services they need and deserve. That does not include adolescents and young adults. Needless to say, this leaves parents and families struggling to make ends meet emotionally and financially to care for their loved ones.
When families have access to the resources and information necessary to gain the support and services they need, they are able to save money, have less stress, and spend more valuable time with their loved ones.
To begin receiving services, families and people receiving supports are required to complete the Intake and Eligibility process, outlined in the steps below.
From the initial steps of gaining eligibility to directly enrolling in specific services, we will help you determine which services best meet your needs.
Step 1: Eligibility Status
Step 2: Front Door Process
Step 3: Enrolling in Medicaid and Service Placement
