Family Forum: Overview of the Shift from MSC to Health Home Care Management

The following information sessions will help inform people receiving services (Medicaid Service Coordination and/or Plan of Care Support Services through OPWDD) and their families/advocates about the upcoming changes to how services will be delivered and reimbursed starting July 2018.
What You Should Know:
OPWDD is changing the way MSC services are delivered and reimbursed to emphasize a more person-centered, integrated care, and comprehensive service coordination. These changes will impact people who receive Plan of Care Support Services and/or Medicaid Service Coordination services (MSC). Beginning July 2018, MSC will become “Health Home Care Management” and Plan of Care Support Services will become “HCBS Care Management”. Both types of Care Management services will be delivered by a new type of organization, called a Care Coordination Organization/Health Home (CCO/HH). The CCO/HH that PCCS has partnered with is Advance Care Alliance of NY (ACANY).
What is most important to remember is: this transition will NOT impact any of the other OPWDD-funded services a person may currently receive, such as living in a certified residence, day habilitation, respite, community habilitation, supported employment, etc.
